Humans are fallible and are prone to making mistakes. Pride, shame and potential loss of face are some of the reasons that people don’t admit to their mistakes. We often lack the knowledge, training, skills and experience to do tasks satisfactorily and thus make mistakes. In the business world people are rewarded for success and penalised for failure. Mistakes are often due to system failures, lack of personal expertise, inefficiencies and inadequacies. In addition, businesses may be operating to out-of-date and incorrect models and theories.

Samuel A Malone is a self-employed training consultant, lecturer and author. He is the author of 21 books published in Ireland, the UK and abroad on learning, personal development, study skills and business management. Some of his books have gone into foreign translations and second editions. He has an M.Ed. with distinction (in training and development) from the University of Sheffield and is a qualified Chartered Management Accountant (ACMA), Chartered Global Management Accountant (CGMA) and a Chartered Secretary (ACIS).
